Forms is a plugin for creating questionnaires that employees complete in the app or intranet. It combines a focused editor with form settings, so you can add and arrange questions and configure how a form behaves without switching tabs. This helps you build forms faster and keep them consistent.
Install the Forms plugin like any other plugin and add it as a block in Pages.
Staffbase offers two Forms plugins that run independently in the Studio: the plugin described here, called Forms, and the previous plugin, now called Classic Forms. See Choosing Between Forms and Classic Forms to decide which to use. To upgrade from Classic Forms to Forms without issues, allowlist the URLs of both plugins.
Top Menu
The top menu of the Form Editor gives you access to the essential form actions:
- Title and description: Update the form title and description at any time.
- Languages: Add or remove language versions for multi-language forms.
- Editor: The main canvas where you create and arrange the questions of your form. Edits reflect the final form layout.
- Preview: Preview your form for desktop and mobile before publishing.
- Responses: View, manage, and export employee submissions.
- Save: Save changes while working on a draft or published form. If the form is already published, Save stores your changes as a draft. They do not take effect until you select Update.
- Publish or Update: Publish a new form or update a published form.
Actions Menu
You can open the actions menu for a form in the Forms overview or in the top menu of the Form Editor. Select the three dots to access additional form-level actions:
- Show form: See how the form looks in the app or intranet. Available only after the form is published.
- Publish or Unpublish: Publish or unpublish the form, depending on its current status.
- Settings: Manage visibility and editor access, as well as form-specific settings such as webhooks and submission rules.
- Copy: Create a copy of the form.
- Copy link: Copy the link of the form to paste into a page or news post.
- Move: Move the form to another space.
- Delete Form: Remove a form that is no longer relevant.
Side Panel
The side panel gives you access to the tools you need while building a form, without leaving the Editor:
- Fields: Open the Fields flyout to add the different types of questions and fields to your form.
- Undo: Undo an action.
- Redo: Redo an action.
