The Form Editor provides an intuitive, focused workspace for creating a questionnaire that employees can complete in the app or intranet.
Add and Configure Fields
A form is built as a sequence of questions rather than a nested layout. When you open a new form, the Editor is an empty canvas ready for your first question. This lets you start adding content right away. Every question you add appears in sequence, in the order employees answer them.
Fields Navigation Bar
To start building your form, open the Fields flyout from the left side panel. The Fields navigation bar gives you an overview of your entire questionnaire and lets you add, move, and delete questions.
- Add field: Select this option at the bottom of the Fields navigation bar and choose from the list of field types.

Move: Available when more than one question exists. Reposition a question at any point during the creation process by:

Dragging it using its drag handle.

Selecting Move up or Move down.

- Duplicate: Copy a question when you want to reuse its configuration. Edit the field text of the copy to distinguish it from the original.
- Delete: Remove a question you no longer need.

Field Canvas
The field canvas shows one question at a time. Use it to write your question and configure its settings.
Each type of field has a clear purpose:
- Intro and end text: Optional messages that bookend the form. Add an introduction with text and an image at the start, and a custom confirmation message that displays after submission.
- Questions: The core building block of a form. Each question has a type, a label, and its own settings: a description, whether it is required, and, for supported types, a conditional logic rule.
- Answer options: Reside inside single choice, multiple choice, recipient, and dropdown questions, and define the list of options a respondent can choose from. You can add or remove options at any time.
When you select a field, the contextual toolbar displays under it with actions relevant to that question:
Select the plus icon to add a new field below the current one, and choose its type before you configure it. You can add as many questions as needed to build out your questionnaire. For single choice, multiple choice, or dropdown questions, add your answer options after choosing the type.
Select the pencil icon to define the question settings. The settings you see vary according to the question type you have selected.
The following settings apply to all question types:
- Description: Add instructions or relevant information for employees.
- Required: Activate this option so employees must answer the question before continuing.
- Images: Add an image to the question to provide additional context for employees.
The following settings apply to specific question types:
- Answer options (choice questions only): Randomize the order of the answer options, add an Other option that lets employees add free text to their answer, or add a None option.
- Conditional logic: Available for single choice, dropdown, recipient, and rating questions. It determines whether the question is shown based on how an earlier question was answered.
Select the trash icon to delete the field.
Finalize and Publish Your Form
The Form Editor supports a flexible workflow that lets you save changes, publish a new form, and update a published form without immediately affecting what employees see. When you are ready with the updates to your form, use one of the following options:
While making updates to your form, use the Save button to store your changes as a draft. Drafts are not visible to employees in the app or intranet.
Review your form before publishing it. The Preview option shows you exactly what employees see on desktop and mobile, and lets you confirm that any conditional logic behaves as expected.
If you have added conditional logic to your questionnaire, use the side navigation panel in the Preview to check how the questionnaire flow looks.
Review the settings of your form, including the space your form is published in, any webhooks, notifications, and who can see it.
You see this option only the first time you publish the form. Once you have reviewed the content and configured the settings, use the Publish button to make the form available to employees according to its visibility. You can also send a push notification to the notification center of your employees when the form is published.
You see this option if the form has already been published. Saved changes do not take effect in the live form until you select Update. Once you have confirmed the updates, use the Update button to apply them. Edits take effect for future submissions only. Past submissions stay unchanged.
