Form settings let you control who can see a form, how submissions are handled, and how employees are notified, so each questionnaire serves its data-collection goal and audience.
To open the form settings:
- In the Studio, navigate to Content > Forms.
- Select the three dots next to the form to open the actions menu, then select Settings.
Alternatively, open the form in the Editor, select the three dots in the top menu, then select Settings.
The form settings open. You can now configure the following settings.
Configuring Title and Description in Multiple Languages
You can configure the title and description of a form in every language that is activated for your organization. If you need additional languages, contact your administrator. For more information, see Creating Multi-Language Questions in the Form Builder for Forms.

In the form settings, select the plus icon to add languages.
The Add Languages dialog opens. - Select the languages you want to add, then select Apply.
- Select Add.
- In the Name field of each language, enter the title of the form.
- Optionally, in the Description field, enter a description to add context to the form.
- Select Save.
You have configured the language settings of the form.
Configuring the Common Settings
The Common Settings show who created the form, the space it belongs to, and the icon used for the form. You can customize the icon in the menu settings.
- In the form settings, navigate to Common Settings.
- Under Show in the menu on, select the device types on which the form displays in the menu.
- Under Show in Breadcrumb, select how the form displays in the breadcrumb:
- Show only when in the desktop menu
- Always show
- Never show
- Activate or deactivate Show progress bar.
- Select Save.
You have configured the common settings of the form.
Configuring Submission Rules
Submission Rules control how many times an employee can respond and whether they can look back at what they submitted.
- In the form settings, navigate to Submission Rules.
- Activate One Response Per Person to limit each employee to a single submission. By default, this option is deactivated and there is no limit.
- Activate Let People Review Answers to let respondents revisit their submission.
- Select Save.
You have configured the submission rules of the form.
Configuring Respondent Data
Respondent Data controls which personal and profile information is captured with each submission.
- In the form settings, navigate to Respondent Data.
- Activate Confidential Responses to hide the personal data of respondents from the results. By default, this option is deactivated and submissions are identified.
- Under Responder Details, select the profile fields, such as department or location, that are collected automatically with every response. You can select system profile fields and custom profile fields specific to your organization. By default, first and last name are collected. Respondents do not see these fields. They appear only in the responses.
- Select Save.
Responder Details is deactivated automatically when Confidential Responses is activated. Your existing selections remain unchanged.

Configuring Webhooks
A webhook automatically sends submission data to an external system each time an employee submits the form, so editors do not need to export and import it manually. Use a webhook when submissions feed another system. When a person needs to act on a submission, use notifications or a recipient question instead.
Staffbase sends the submission data as a POST request with a JSON body to your HTTPS endpoint, and signs each request with a secret that is generated for the webhook. The receiving system uses the secret to verify that the request came from your Staffbase environment.
- In the form settings, navigate to Webhook.
- Select Add Webhook.

Enter the HTTPS endpoint of the external system that receives the submission data, then select Add Webhook.
The URL must start with https://. URLs starting with http:// are not supported. - Select Copy Secret and enter the secret in the settings of the receiving system.
- Activate the Webhook toggle. By default, a new webhook is deactivated.
- Select Save.
You have set up a webhook for your form. Every submission is now sent automatically to the endpoint you provided. You can edit, test, or delete the webhook at any time from its actions menu. For the payload structure and how to verify the signature, see Configuring Webhooks for Forms.
Configuring Notifications
Notifications control who is informed about a new submission and how much of it they see. If your questionnaire has a recipient question, the users selected in that question receive an email notification when the matching answer is submitted. Otherwise, add recipients here to notify them of every response.
- In the form settings, navigate to Notifications.
- Activate Include Answers in Email to send the full response content with each notification instead of only a notice that a submission was received. This also applies to notifications sent to the recipients of a recipient question.
- Under Recipients, add one or more users who are notified every time the form receives a response.
- Select Save.

Configuring Visibility in the App
You can control who sees a form in the app or intranet, based on specific users, groups, and API tokens, when you create the form or at any time later.
- In the form settings, navigate to Visibility in the App.

From the dropdown, select the visibility of the form.
If you select For selected space users and groups, additional options display. - Select the options relevant for the form, then use the corresponding dropdowns to narrow down your audience.
- Select Visible in the Public Area if the form must be visible in the public area.
- Select Save.
You have configured the visibility of the form in the app or intranet.
Configuring Editors
You can specify which users with content-specific rights are allowed to edit the form. Editors receive an email notification when another editor publishes or unpublishes the form.
- In the form settings, navigate to Editors.
- Select one or more options to narrow down the editors, then use the corresponding dropdowns to specify the editors.
- Select Save.
You have assigned the editors who can access and edit the form.
