Forms lets you build structured questionnaires that collect information from employees and route their answers wherever you need them.
Staffbase offers two Forms plugins that run independently in the Studio: the plugin described here, called Forms, and the previous plugin, now called Classic Forms.
When creating a form, write the description the way you would explain the form to a colleague: plain and specific about who it is for, what triggers it, and what result it produces. You can revise it at any time, even after the form is published. For example, the description of a safety incident report might read: "Relevant to our frontline workforce, especially those working in our factory locations. Used to record workplace accidents, injuries, equipment damage, or near-misses on the factory floor to ensure safety and compliance, and to prevent future issues."
Prerequisites
- You have installed the Forms plugin.
Creating a Form
- In the Studio, navigate to Content > Forms.

Select Create a new Form.
The Create a Form dialog opens. - Enter a title that clearly describes the purpose of the form.
- Enter a short description of the form.
- From the space dropdown, select a space.
- Set the visibility of the form.
- Select Create.
The Editor opens, where you can add and arrange your questions.
You have created a form as a draft. Drafts are not visible to employees in the app or intranet. You can access the form at any time from the Forms overview. Continue with Building Your Form Using the Form Editor.
